BỆNH XẢY THAI TRUYỀN NHIỄM TRÊN CHÓ (CANINE BRUCELLOSIS)

1. Căn bệnh

Do Brucella canis là vi khuẩn G-, hiếu khí. Kích thước nhỏ (1.0 đến 1.5 μ m). Hình thái khuẩn lạc thô do nồng độ thấp của LPS nhẵn (lipopolysaccharide – LPS). Brucellae ổn định trong môi trường lên đến 2 tháng với sự có mặt của nhiệt độ lạnh và các mảnh vụn hữu cơ. B. canis tồn tại ngắn bên ngoài cơ thể, bất hoạt bởi chất khử trùng: iodophors hoặc các hợp chất amoniac bậc bốn và ánh sáng mặt trời.

  1.  Sự lây nhiễm

Chó và họ động vật có vú ăn thịt, bao gồm chó sói, chó rừng và cáo được phát hiện là nhạy cảm. Mèo, thỏ và động vật linh trưởng dễ bị nhiễm trùng thực nghiệm. Người bị nhiễm do tiếp xúc với những con chó bị nhiễm bệnh, hoặc những mẫu bệnh phẩm mà không có bảo hộ, nhưng người tương đối đề kháng với bệnh.

Brucella về giống gồm có sáu loài cổ điển dựa trên lựa chọn vật chủ và phân tích di truyền: Brucella abortus (trâu bò), Brucella canis (chó), Brucella melitensis (dê, cừu), Brucella neotomae (loài gặm nhấm), Brucella ovis (cừu) và Brucella suis và biovars của nó (heo, trâu, bò, thỏ rừng, động vật gặm nhấm, móng guốc và hoang dã ). Brucella mới (Brucella Ceti và Brucella pinnipedialis) đã được phân lập từ động vật biển có vú và động vật có vú chuyên ăn thịt thủy sản như hải cẩu, hải mã.

3. Cách sinh bệnh

Lây truyền tự nhiên bởi nhiều con đường. Chó cái bị nhiễm truyền B. canis trong động dục, ở giao phối hoặc sau khi sẩy thai thông qua tiếp xúc giữa miệng, lưỡi với dịch âm đạo thú bệnh. Lây truyền phổ biến nhất là do tiếp xúc của miệng, lưỡi với các chất từ thai sảy. Có thể truyền qua sữa, nhau thai. Tinh dịch và nước tiểu đã được cho là nguồn lây nhiễm từ con đực chứa vi khuẩn trong tuyến tiền liệt và mào tinh hoàn của chúng. Những đường lây khác: truyền qua dụng cụ nội soi âm đạo, truyền máu, thụ tinh nhân tạo và sử dụng ống bơm, kim tiêm bị nhiễm bệnh. Tỉ lệ nhiễm thay đổi theo tuổi của gia súc, điều kiện chuồng trại, giống, và vị trí địa lý.

4. Triệu chứng bệnh sảy thai truyền nhiễm trên chó

Chó lười vận động. Chó đực phì đại tinh hoàn, viêm da bìu, teo 1 hoặc 2 bên tinh hoàn, liệt chi sau. Chó có hạch lympho ngoại vi sưng to, nồng độ globulin trong máu cao thời gian dài. Chó cái sảy thai, chảy máu dịch âm đạo màu xanh hoặc xám. Chó con chết từ ngày 45 – 60 của thai kỳ. Chó con phù nề dưới da, tắc nghẽn và xuất huyết da bụng, thai chết trong tử cung một thời gian trước khi sảy thai. Dấu hiệu lâm sàng chính của bệnh là sảy thai xảy ra bất cứ lúc nào sau khi phối giống, vô sinh. Viêm tủy xương hoặc viêm đa khớp của xương chi, gây què quặt.

5. Bệnh tích

Viêm da tạo nhiều ổ mủ mãn tính giống như bệnh tích viêm da liếm (hình trên). Bệnh tích mắt bao gồm viêm màng bồ đào trước, bệnh tăng nhãn áp thứ cấp, bong võng mạc, viêm tròng trắng và võng mạc, viêm dây thần kinh thị giác, viêm nội nhãn với bệnh tăng nhãn áp thứ cấp hoặc là sự teo lại, mất chức năng của mắt và phù giác mạc với quá trình kéo mây hoặc đục mắt. Để lại sẹo liên quan đến tròng trắng và võng mạc của mắt (tăng phản ứng có thể được quan sát sau khi điều trị những con chó với viêm màng bồ đào). Lách phì đại, phì đại hạch bạch huyết. 6. Chẩn đoán bệnh sảy thai truyền nhiễm trên chó

Kết quả xét nghiệm lâm sàng: tăng globulin huyết và giảm albumin huyết (mãn tính), dịch hút hoặc mẫu sinh thiết từ hạch bạch huyết cho thấy phì đại hạch bạch huyết. Tăng lympho bào dịch não tủy, tăng nồng độ protein. Phim X – quang cho thấy nhiễm trùng đĩa đệm.

Kiểm tra tinh dịch: bất thường thấy rõ từ tuần 5 – 8 sau khi nhiễm bệnh, Bất thường bao gồm tinh trùng chưa trưởng thành, phần đầu của tinh trùng biến dạng, phần giữa sưng lên, và giữ lại những giọt nguyên sinh. Ở tuần 15, đuôi uốn cong, đầu tách ra và ngưng kết phần đầu được nhìn thấy. Tập hợp lớn các tế bào viêm, thường bao gồm bạch cầu trung tính, bao quanh đại thực bào chứa tinh trùng kết dính theo lối thực bào. Hơn 90% số tinh trùng bất thường ở 20 tuần. Không tạo tinh trùng tương ứng với teo tinh hoàn hai bên trên chó đực

Xét nghiệm huyết thanh học: 2-mercaptoethanol (ME) kiểm tra ngưng kết nhanh trên phiến kính (RSAT; ME RSAT) được ưu tiên như là một quy trình kiểm tra tại phòng bởi vì không tốn kém, nhanh chóng và đặc hiệu; nó cũng dùng phát hiện kháng thể sớm. Phản ứng ngưng kết ống (TAT) cũng được dùng để xác định sự lây nhiễm, tuy nhiên, nó thường cho kết quả dương tính giả. Miễn dịch khuyếch tán trên thạch (AGID) kháng nguyên vách tế bào soma, rất nhạy cảm (khoảng 5 – 10 tuần sau nhiễm, tuy nhiên qui trình phức tạp. kháng nguyên protein bên trong tế bào chất (CPAg)-AGID là test kiểm tra cụ thể nhất, giúp khẳng định các trường hợp nhiễm bệnh mãn tính; tuy nhiên, xét nghiệm này ít nhạy cảm nếu nhiễm bệnh thời gian đầu. Elisa có kết quả tốt đối với các chủng khác nhau; tuy nhiên, xét nghiệm này đòi hỏi độ tinh khiết của kháng nguyên.

7. Điều trị

Brucella sống ký sinh nội bào nên lưu ý dùng thuốc kháng sinh phải tan trong lipid, thâm nhập vào mô và đạt nồng độ nội bào cao (Doxycycline, minocycline). Không điều trị bằng 1 phát đồ duy nhất. Nên kết hợp 3 – 4 loại thuốc: làm tăng hiệu quả điều trị, tránh tái phát. Ở người, chó: Brucella giảm tính nhạy cảm với cephalosporin, acid nalidixic và cycloserine. Nếu xét nghiệm máu 2 lần (-) với B. canis, sau 3 tháng có thể ngừng điều trị.

Minocycline/PO/ liều cao. Hoặc doxycyclin + streptomycin /IM: cho hiệu quả cao nhất. tetracycline + quinolone + aminoglycoside + sulfonamides: có tác động hiệp lực. Quinolone (ciprofloxacin , norfloxacin , enrofloxacin): Hiệu quả ở bệnh mãn tính, nhiễm trùng mắt, nhiễm trùng đĩa đệm, ngừa sảy thai. Tetracycline, chloramphenicol, aminoglycoside, spectinomycin, rifampin, cephalosporin thế hệ thứ ba, ampicillin, sulfonamides potentiated và quinolone: cho hiệu quả trung bình. Có thể dùng: tetracycline + streptomycin: Điều trị: 14 – 28 ngày/ chó nhiễm bệnh 1 – 2 tháng. Tigecycline + aminoglycosid cho hiệu quả tương đối với B. melitensis.

8. Phòng bệnh

Chó giống: kiểm tra huyết thanh 3 – 4 tuần trước khi giao phối. Phòng ngừa được thực hiện bằng cách kiểm dịch đối với tất cả các nơi mới thu mua cho đến khi hai kết quả kiểm tra huyết thanh trong khoảng thời gian 1 tháng là âm tính. Bệnh không có vaccin phòng, nên cần kiểm soát sự lây nhiễm trong trại chó bằng cách khử trùng các đồ dùng cho thú, kiểm tra huyết thanh học, loại bỏ kịp thời thú mắc bệnh. Người làm việc với những chó bị nhiễm bệnh và chất thải của chúng hoặc dịch tiết nên đeo găng tay cho đến khi các kiểm tra huyết thanh được thực hiện để xác định mức độ nhiễm trùng. Thú nhập đàn cho đến khi bệnh được loại trừ.

Sự di chuyển của chó trong một đàn cũng nên được giới hạn, loại bỏ các trường hợp bệnh đã được chứng minh hoặc nghi ngờ đến khu cách ly. Sự nhập đàn mới có nguy cơ cao bị nhiễm bệnh nên loại bỏ, đặc biệt là nếu chó không có lý lịch đầy đủ. Thú bệnh không được thả ra để bán hoặc cho bất cứ mục đích nào khác vì chúng có thể lây bệnh cho thú khác và cho công đồng. Nếu chó bệnh được giữ lại như là thú cưng, chó làm việc thì việc điều trị là cần thiết, chúng nên được thiến và chuyển đến nhà ở riêng biệt.

9. Sức khỏe cộng đồng

Bốn trong số các loài Brucella gây bệnh ở người: B. abortus, B. suis, B. melitensis, và B. canis. Brucella spp… khác nhau về tính độc hại của chúng đối với con người, nhưng tiếp xúc với thú vật hoặc các mô hoặc dịch tiết có nguy cơ nhiễm bệnh. Bệnh brucella là bệnh nhiễm trùng, đặc biệt trong phòng thí nghiệm được báo cáo nhiều nhất. Bác sĩ thú y cũng có thể bị nhiễm theo hình thức tiêm vắc-xin nhược độc đối với B. abortus hoặc B. melitensis (khí dung qua mắt). Độc lực cao nhất được ghi nhận với B. melitensis và biovars của B. suis, trong khi B. abortus có độc lực vừa phải. Khi bị nhiễm trùng có khả năng dẫn đến viêm đa khớp, viêm kết mạc, viêm màng bồ đào và là một trong những biểu hiện phổ biến của nhiễm trùng do Brucella. Biến chứng hiếm gặp bao gồm giảm tiểu cầu, viêm nội tâm mạc van tim, viêm màng não, viêm phổi, viêm thần kinh, viêm cầu thận, viêm gan, và áp-xe nội tạng.     I have virtually no expertise in programming however I was hoping to start my own blog in the near future.
